    POSITION SUMMARY: 

    Under the supervision of the Support Services Program Manager and in collaboration with the Health and Wellness Team, the Case Worker will provide ongoing client engagement to encourage participation in support services. The Case Worker is responsible for completing biannual assessments with clients through completion of a care plan including other linkages and access to determinants of health along with compliance with medical care and adherence to HIV medications. The Case Worker will also support community building through planning and implementing social programs, community advisory boards (CABs), and workshops. They are responsible for helping to develop, implement, monitor, and evaluate all health and social support services primarily for LGBTQA+ communities including group facilitation, training, community outreach, social media recruitment, and event planning. 

    ESSENTIAL DUTIES:

    • Meet with clients regularly and document progress and strengths. 

    • Provide case management services to clients including assisting clients access routine medical care for HIV. 

    • Supports client advocacy and assistance in accessing services as needed. 

    • Serve as the program evaluator. 

    • Manage data collection tools (i.e. Survey Monkey, Google Forms, Eventbrite, WelTel, LeO, etc.) to provide timely reports on program participant attendance, retention, and satisfaction. 

    • Present quarterly reports on behavioral health services, support services and integrated services, or as needed; 

    • Collect feedback and present insights from, behavioral health services, support services and integrated services including peer support groups, community development programs and psychoeducational workshops, or other programs as needed; 

    • Oversee overdose prevention programs including educational workshops, tracking overdose interventions inventory and distributions, and the development and implementation of educational curriculum on best practices 

    • Enter monitoring data and units of service in a timely manner into Weltel, and other databases. 

    • Keep all client information, data, and files up to date and accurate in compliance with grant expectations. 

    • Maintain client medical and service record files and be responsible for compliance and completeness. 

    • Conducts outreach to identify and engage individuals for HIV services. 

    • Provide documentation that establishes an individual’s eligibility for services and the provision of services. 

    • Conduct initial comprehensive assessment(s) of client’s needs and support systems. 

    • Collaboratively develop and implement a comprehensive individual service plan with the client. 

    • Regularly assess the efficacy and implementation of the service plan, updating and reevaluating as necessary. 

    • Properly manages and logs all HIV testing and other program-related incentives to eligible clients. 

    • Provide support for client’s compliance and adherence with medications. 

    • Manage incentive tracker and distribution. 

    • Refer to other social service agencies, when appropriate. 

    • Coordinate, implement, and facilitate on-site and off-site social and community groups, personal and educational development workshops, and wellness programs regularly. 

    • Coordinates and implements at least one (1) HIV testing event for a minimum of 30 individuals. 

    • Develops, maintains, and collaborates with stakeholders and REACH LA’s community partners, including the LGBTQ+ community, mental health providers, substance abuse counselors and treatment centers, Los Angeles County Division of HIV and STI Programs, and other community-based agencies, in facilitating HIV prevention efforts as well as other health education workshops, harm reduction strategies, and provides resources through a status neutral approach; and 

    • Regular and reliable attendance with punctuality is essential when working with the public. 

    • Other duties as assigned by the program manager. 

    OTHER RESPONSIBILITIES:

    • Participates in professional development and networking conferences and events, as needed 

    • Updates and maintains weekly schedules and timesheets.

    JOB SPECIFICATIONS: (Knowledge, skills, abilities, and experience generally required for competent performance) 

    Education and Experience 

    • Bachelor’s degree or equivalent required; in public health, health science, sociology, social work, or similar field from an accredited program. 

    • Minimum two (2) years of experience working with young LGBTQIA and non-gay identified African American and Latinx MSM living with HIV. 

    • Experience working with individuals with mental health diagnoses, living with HIV and substance use disorders strongly preferred; 

    • Experience providing direct service to individuals living with HIV or experience providing HIV/AIDS-related social services preferred; 

    • Experience in the following areas strongly preferred: outreach and engagement strategies, intensive case management services, best practice models, mental health and substance use disorder service, crisis intervention, suicide assessment and prevention, and mandated reporting; and 

    • Knowledge of motivational interviewing techniques, strength-based case management tools, and utilizing a trauma-informed approach. 

    • Experience working effectively with diverse communities preferred. 

    Knowledge, Skills, and Abilities 

    • Bilingual English/Spanish preferred 

    • Knowledge of the following: Trauma-informed care approach; Health disparities spectrum of HIV and STIs; harm reduction principles; human sexuality; substance use disorders; motivational interviewing techniques; health and social determinants of gay and non-gay identified MSM; 

    • Understanding of case management practices and ability to timely, accurately, and clearly document case notes. 

    • Maintain confidentiality according to HIPPA and other privacy laws; 

    • Ability to work efficiently and effectively independently and on a team with diverse community members and staff to work toward common goals. 

    • Strong written, verbal, and interpersonal skills; 

    • Understanding of conflict resolution and de-escalation interventions 

    • Proficient in Google Workspace and Microsoft Office Suite software. Familiarity with Notion is a plus, but not required. 

    • Good knowledge of the following: Health disparities spectrum of HIV and STI disease; harm reduction theory; behavior modification theory; human sexuality, substance use and abuse, motivational interviewing techniques; health and social concerns of diverse populations of gay and non-gay identified MSM; and venues where the target population congregates, including the Internet and social media platforms or willingness to learn. 

    • Strong team player able to work effectively and efficiently with diverse community members and employees toward common goals and work independently. 

    • Operates standard office equipment. 

    • Able to work some evenings and weekends (including all local pride events) 

    LICENSURE AND CERTIFICATIONS: 

    • Valid driver’s license and liability insurance if driving a personal vehicle on REACH LA business or reliable transportation. 

    • HIV Testing Certification required or must pass the HIV Testing Certification course provided by DHSP within 6 months of employment. Salary is guaranteed upon completion of HIV Certification within 180 days of employment.

    POSITION SUMMARY: 

    Under the supervision of the Support Services Program Manager, the Program Coordinator will provide an additional layer of oversight of program compliance by maintaining alignment of program service provisions with all organizational and grant-specific deliverables. A key responsibility of the Program Coordinator will be to provide continuous program evaluation through data collection, management, and analysis of various metrics in behavioral health services, support services and integrated services. 

    ESSENTIAL DUTIES: 

    • Oversee provision of case management services at RLA through regular review of program data and quality assurance procedures. 

    • In collaboration with case workers, coordinate direct referrals and linkages to behavioral health services, support services, and integrated services. 

      ▪ Review all participant assessments including need for services, eligibility for services, collaborative case plans, mental health and/or substance use-related questionnaires. 

      ▪ Coordinate and facilitate case consultations and program meetings, as needed. 

      ▪ Review case notes for completion, accuracy, and compliance with all grant guidelines. 

      ▪ Provide training relevant to ICMS, best practice models, mental health, substance use disorder, crisis intervention, suicide assessment and prevention, trauma-informed care, harm reduction, etc., as needed 

      ▪ Provide engagement and retention services to clients. 

      ▪ Supports client advocacy and assistance in accessing services as needed. 

      ▪ Serve as the program evaluator, when needed. 

      ▪ Provide monthly statistics on program data including participant enrollment or discharge, program attendance, retention, and satisfaction.
