Position Summary:
Under the supervision of the Program Manager, the Programs Coordinator supports the planning, coordination, and implementation of youth-centered and youth-led programming serving 2S/LGBTQIA+ youth ages 12–26. This role is responsible for implementing Elevate Youth California (EYC)–funded youth development, leadership, and policy engagement initiatives in alignment with the organization’s approved work plan. The Programs Coordinator creates partnerships with schools and community-based organizations and supports youth engagement in policy education, advocacy, and systems-level change. The Programs Coordinator works collaboratively with youth, educators, families, and community partners to advance inclusive school environments, affirming policies, and equitable access to resources, while fostering leadership development, prevention, and community wellness. The Programs Coordinator plays a lead role in coordinating, facilitating, and overseeing delivered in schools and community spaces for youth, educators, and community partners.
POSITION RESPONSIBILITIES
Coordinate and support youth development programs that include leadership development, policy education, advocacy activities, creative projects, and community-based initiatives.
Support youth engagement in understanding and navigating school- and community-based policies that impact 2S/LGBTQIA+ students, including equity, safety, and access to services.
Lead implementation of grant-funded initiatives, including Elevate Youth California (EYC), aligned with approved work plans and timelines
Coordinate trauma-informed prevention, mentorship, and youth leadership programming in school and community settings.
Build and maintain collaborative partnerships with schools grade 6-12, colleges/universities, school staff, and district or campus-based stakeholders.
Serve as a liaison between the organization, youth participants, and school or community partners to support program implementation and alignment.
Serve as the primary facilitator for youth, staff, and partner trainings related to youth development, equity, prevention, leadership, and school climate.
Coordinate training schedules, content development, materials, and logistics in collaboration with school partners and internal teams.
Support youth-led policy, systems, and environmental (P.S.E.) change initiatives in collaboration with community partners.
Assist youth advisory groups and leadership bodies in providing feedback that informs organizational programs, policies, and partnerships.
Facilitate Listening Sessions and other youth feedback processes required by funders
Support outreach, recruitment, and retention efforts in school and community settings.
Collaborate cross-departmentally to align programming with organizational goals related to equity, prevention, and systems change.
Maintain accurate program schedules, documentation, and required records.
Link target youth into Mental Health services, and HIV/STI testing services.
Participate in staff meetings, trainings, and professional development related to youth development, education systems, and policy.
Contribute to program reflection, documentation, and continuous improvement efforts.

POSITION QUALIFICATIONS
Education & Experience
Bachelor’s degree in public health, social sciences, data management, evaluation, or a related field, or equivalent experience.
Minimum two (2+) years of experience working with youth, preferably in school-based, community-based, or policy-informed settings.
Experience collaborating with schools, districts, or educational institutions.
Experience supporting or facilitating youth engagement in policy, advocacy, or civic education is preferred.

POSITION SUMMARY:
Under the supervision of the Program Manager, and in collaboration with the Programs and Data Specialist, the Engagement Specialist supports youth-centered and youth-led programming serving 2S/LGBTQIA+ youth ages 12–26. This position assists with program coordination, event planning, administrative support, and engagement activities across school- and community-based settings. The Engagement Specialist plays a key role in supporting timely event request submissions, program logistics, and documentation while providing direct support during youth programs and events. The Engagement Specialist also supports the coordination and delivery of trainings in schools and community spaces and assists with logistics and engagement during learning activities. POSITION SUMMARY:
Under the supervision of the Program Manager, the Data Specialist supports data collection, tracking, and evaluation across all Youth Development programs. This position strengthens organizational systems by ensuring accurate information is collected and maintained to allow for an agile work environment that encourages creative changes to initiatives that will lead to a better distribution of resources to clients. The Data Specialist must also manage processes in line with funding expectations and programmatic goals. In addition to evaluation and systems support, this role assists with the occasional facilitation of training and learning activities delivered across Youth Development programs. This role is responsible for data collection and reporting of Elevate Youth California (EYC)–funded youth development, leadership, and policy engagement initiatives in alignment with the organization’s approved work.

POSITION SUMMARY:
Under the supervision of the Support Services Program Manager and in collaboration with the Health and Wellness Team, the Case Worker will provide ongoing client engagement to encourage participation in support services. The Case Worker is responsible for completing biannual assessments with clients through completion of a care plan including other linkages and access to determinants of health along with compliance with medical care and adherence to HIV medications. The Case Worker will also support community building through planning and implementing social programs, community advisory boards (CABs), and workshops. They are responsible for helping to develop, implement, monitor, and evaluate all health and social support services primarily for LGBTQA+ communities including group facilitation, training, community outreach, social media recruitment, and event planning.
ESSENTIAL DUTIES:
Meet with clients regularly and document progress and strengths.
Provide case management services to clients including assisting clients access routine medical care for HIV.
Supports client advocacy and assistance in accessing services as needed.
Serve as the program evaluator.
Manage data collection tools (i.e. Survey Monkey, Google Forms, Eventbrite, WelTel, LeO, etc.) to provide timely reports on program participant attendance, retention, and satisfaction.
Present quarterly reports on behavioral health services, support services and integrated services, or as needed;
Collect feedback and present insights from, behavioral health services, support services and integrated services including peer support groups, community development programs and psychoeducational workshops, or other programs as needed;
Oversee overdose prevention programs including educational workshops, tracking overdose interventions inventory and distributions, and the development and implementation of educational curriculum on best practices
Enter monitoring data and units of service in a timely manner into Weltel, and other databases.
Keep all client information, data, and files up to date and accurate in compliance with grant expectations.
Maintain client medical and service record files and be responsible for compliance and completeness.
Conducts outreach to identify and engage individuals for HIV services.
Provide documentation that establishes an individual’s eligibility for services and the provision of services.
Conduct initial comprehensive assessment(s) of client’s needs and support systems.
Collaboratively develop and implement a comprehensive individual service plan with the client.
Regularly assess the efficacy and implementation of the service plan, updating and reevaluating as necessary.
Properly manages and logs all HIV testing and other program-related incentives to eligible clients.
Provide support for client’s compliance and adherence with medications.
Manage incentive tracker and distribution.
Refer to other social service agencies, when appropriate.
Coordinate, implement, and facilitate on-site and off-site social and community groups, personal and educational development workshops, and wellness programs regularly.
Coordinates and implements at least one (1) HIV testing event for a minimum of 30 individuals.
Develops, maintains, and collaborates with stakeholders and REACH LA’s community partners, including the LGBTQ+ community, mental health providers, substance abuse counselors and treatment centers, Los Angeles County Division of HIV and STI Programs, and other community-based agencies, in facilitating HIV prevention efforts as well as other health education workshops, harm reduction strategies, and provides resources through a status neutral approach; and
Regular and reliable attendance with punctuality is essential when working with the public.

JOB SPECIFICATIONS: (Knowledge, skills, abilities, and experience generally required for competent performance)
Education and Experience
Bachelor’s degree or equivalent required; in public health, health science, sociology, social work, or similar field from an accredited program.
Minimum two (2) years of experience working with young LGBTQIA and non-gay identified African American and Latinx MSM living with HIV.
Experience working with individuals with mental health diagnoses, living with HIV and substance use disorders strongly preferred;
Experience providing direct service to individuals living with HIV or experience providing HIV/AIDS-related social services preferred;
Experience in the following areas strongly preferred: outreach and engagement strategies, intensive case management services, best practice models, mental health and substance use disorder service, crisis intervention, suicide assessment and prevention, and mandated reporting; and
Knowledge of motivational interviewing techniques, strength-based case management tools, and utilizing a trauma-informed approach.
Experience working effectively with diverse communities preferred.
Knowledge, Skills, and Abilities

LICENSURE AND CERTIFICATIONS:
